CareMatrix is an innovative project addressing the challenges of multimorbidity in aging populations, impacting over 50 million Europeans. It advocates a shift from traditional, diagnosis-specific healthcare towards an integrated, patient-centered approach. Focusing on interoperability of digital health systems, CareMatrix leverages Artificial Intelligence (AI) to enhance care quality and efficiency. The initiative, supported by Horizon 2020, involves key partners developing the INCA Solution - a digital platform for coordinated care management. INCA integrates patient data, supports shared care plans, and features a multidisciplinary approach, placing patients at the heart of healthcare.

Bup Region Skåne, tillsammans med Innovation Skåne, utvecklade under åren 2020–2022 fram den digitala tjänsten Bup Skåne online. En digital en väg in för barn och unga, samt viktiga vuxna, att enkelt kunna hitta kontaktvägar och få tips, råd och hjälp till självhjälp om sin psykiska hälsa. Hela Bup Skåne online tjänsten är framtagen från grunden med tjänstedesign och co-creation tillsammans med målgrupperna; barn och unga, närstående, brukarföreningar, elevhälsa m.fl. Involvering av målgruppen genom hela vägen från behovsanalys för att förstå behoven, testa och prova ut lösningar innan sedan den färdiga ”produkten” lanseras, säkerställer att lösningen motsvarar de behov och förväntningar som målgruppen har.
Vi kommer berätta hur vi har praktiserat barnkonventionen och arbetat med co-creation och målgruppsinvolvering under utvecklingen av Bup Skåne online. Dela lärdomar och erfarenheter för att involvera barn i frågor som rör dem. Att involvera sin målgrupp, oavsett vilken, i sitt innovationsarbete är alltid värdefullt.

Delivery of nursing terminology in the digital age is essential to the provision of safe nursing care at a global, national and local level. The provision is fundamental to the delivery of multi-disciplinary care, where nursing information is critical. The requirement is therefore to ensure that clinical terminology in EHR systems is clinically correct, up to date and available for use. The formal collaboration between the International Council of Nurses (ICN) and SNOMED International seeks to deliver these aspirations, with an ongoing commitment to ensure the alignment of ICNP (International Classification of Nursing Practice) and SNOMED CT. The collaboration will provided clinically ensured nursing terminology within EHRs and that the data is available to support nursing research globally.
