Digital interventions can have beneficial effects on our health and prescribing digital therapies is not in the distant future - for use in institutions and at home. While the benefits are encouraging for patients, next-of-kin, healthcare personnel and institutions, we need a deeper understanding of the nature of digital interventions that are powered by sensors, algorithms and apps. The understanding should help us answer questions like how we verify the clinical benefits of the therapies or if health systems have the expertise, regulations, organisational roles and health economic arrangements in place to adopt digital therapies. This talk reports on some initiatives in Scandinavia and Germany that address the development, testing, regulation and procurement of digital interventions. As a backdrop, the talk uses discussions in the Health Data Innovation Council (https://healthdatainnovation.eu) to highlight different aspects of data-driven health interventions and the implications across the continuum of care (prevention, treatment, rehabilitation and palliative care), health technology life cycle (design, testing, commercialization and operations) and oversight (approval, certification and regulation).

Every year, several million triages are performed in Sweden to evaluate and prioritize patients' medical needs. In the past, these assessments were conducted solely over the phone or at primary care centers, without any means of systematic evaluation. However, a groundbreaking shift is underway as Sweden digitizes its triage process, revolutionizing one of the most crucial processes in healthcare. Now, for the first time ever, patients' questions, responses, and advice can be analyzed systematically. The implications for patient safety and equity are profound, and it is essential to consider how this transformation affects vulnerable groups, such as the elderly and frail, who may not have access to digital tools. Join Jonathan Ilicki on a journey beyond the ordinary and gain unique insights into the pivotal role of triage in coordinating healthcare. Discover how digitization is transforming healthcare delivery in Sweden and shaping the future of medical care.

Within Nordic cooperation and with funding from the Nordic Council of Ministers, a number of projects are underway as part of the Nordic Vision 2030 goals; The Nordic region will become the most sustainable and integrated region in the world. Some examples of ongoing and completed projects are presented during this session. Integrated Healthcare and care (iHAC) 2021-2024 is run by the Centre for Rural Medicine in Storuman Sweden and the Nordic Welfare centre. The project focuses on the citizen perspective in national and regional models for integrated management of healthcare and social care including the data management. Nordforsk runs the more research-oriented project Nordic Commons with a focus on secondary use of health data for research. Nordic Innovation has additional investments related to health data promotion from an innovation perspective.THL in Finland runs the Finnish presidency project 2021-2023, World's Smoothest Cross-Border Mobility and Daily Life Through Digitalisation with a focus on e-prescriptions and Patient Summary. In 2021, the project published the report Baseline study of cross-border data exchange in the Nordic and Baltic countries. The project is partly a continuation of the project the Swedish E-health authority carried out in the Nordic project Care and care at a distance (VOPD) 2018-2021 with a focus on cross border e-prescriptions and patient summaries. This session can be seen as a brief introduction related to what happens within the Nordic Cooperation related to health data.

Equal access to health data is far from reality today and there is a lack of comparison between countries. To motivate, understand and increase progress, we start from the position that highlighting current status, sharing evidence related to practices and policies, and identifying major enablers and obstacles across national and regional boundaries is necessary. Therefore, we have invited panel participants to introduce five different countries, representing five different stages of and approaches to patients’ ORA implementation.

Most Nordic countries have created their own set of FHIR base profiles. There has not been harmonization between these efforts, rather each country has had their own internal process. However, harmonizing the base profiles would be really useful. A harmonized content format across the Nordic countries would allow a FHIR app developed in one country to be used in another without major modifications. It would also simplify sharing health data across borders.A new initiative is being launched to harmonize the FHIR base profiles across the Nordic countries.
