In this workshop I will introduce the basic principles of realist evaluation and encourage you to ‘think like a realist’. The workshop will consider why realist methods are useful in evaluating complex interventions and provide an overview of key concepts in realist evaluation, including context, mechanism and outcome configurations. Using small group work, you will be asked to apply realist thinking to some examples. CareMatrix is an innovative project addressing the challenges of multimorbidity in aging populations, impacting over 50 million Europeans. Focusing on interoperability of digital health systems, CareMatrix leverages Artificial Intelligence (AI) to enhance care quality and efficiency. The initiative, supported by Horizon 2020, involves key partners developing the INCA Solution - a digital platform for coordinated care management. Artificial intelligence-based (AI-based) language analysis is undergoing a “paradigm shift” initiated by new machine learning models, large language models, leading to unprecedented accuracies over most computerized language processing tasks such as web search, automatic machine translation, and automatic question answering. The success of the large language models is mostly attributed to their capability to numerically represent words in their context, long a weakness of popular psychological research techniques that use “bag-of-words” or word count approaches. Large language models can enable psychological language analysis to quantify the meaning behind words and human language in its larger context and thus capture truer psychological states and traits of the person behind the words. We discuss evidence showing that AI's paradigm shift to large language models is the missing piece for a parallel transformation in mental health assessment from the strong reliance on rating scale responses to more accurate, fine-grained, and ecologically valid assessments by fully leveraging patients’ own descriptions. We believe that this is an essential step in a more data-driven healthcare.

In many ways, Sweden was early in its abilities to share data between caregivers and regions. These abilities are mainly achieved through the infrastructure provided by the regions and municipalities infrastructure, which has entailed certain limitations on the national government´s ability to govern, prioritize and take responsibility for the national and international perspective. In recent years the government has increased its efforts, for instance through various assignments to governmental authorities such as the E-health agency.
