Therapeutic alliance is a key predictor of treatment outcomes, with microcounseling skills—such as reflective listening and open-ended questions—playing a vital role. Regular, especially immediate, feedback helps therapists refine these skills, but traditional coding methods are costly and time-intensive. This study explored fine-tuned Large Language Models (LLMs) to automatically identify microcounseling skills in psychotherapy sessions.Using annotated psychotherapy transcripts, the model was trained to classify therapist statements, explain its classifications, and suggest alternative responses. The model demonstrated strong overall performance (Accuracy: 0.74, Precision: 0.81, F1: 0.75), yet faced challenges with more nuanced relational dynamics. The findings highlight the potential of LLMs to provide scalable, automated feedback on therapist skills, demonstrating the feasibility of using digital tools to enhance psychotherapy effectiveness.
