20 year’s experience using digital decision support for chronic disease (HIV) Har passerat

InfCare HIV quality register and decision support tool has now been used nationwide in Sweden and in other Nordic countries for 20 years. In Sweden, a great amount of structured clinical data has been collected during the years and the Swedish HIV physicians have included the decision support system as an integral part of all patient-related workflows in the clinic. The system gives clinicians in their daily practice a holistic view of the patient’s state of health over time as well as real time access to key performance indicators regarding the care given at the clinic.

Data from InfCare HIV has been used in numerous research projects and studies to support developing treatment guidelines and improve care pathways for HIV patients.

Similar treatment monitoring and decision support tools are today used also for other clinical specialties. The different disease specific platforms gather relevant data automatically from the hospitals’ electronic health record systems. The data flows to the platform through integrations, eliminating the manual work done by the healthcare professionals. Only certain quality parameters are entered manually by the clinicians.

Patient related outcome data and quality of life data is reported on the platform by the patients. The patients use the digital service that is fully integrated and synchronised with the disease specific clinical management system and the submitted data can be reviewed by the clinician in real time. All data is gathered in a common database and can be utilised for reports, research, benchmarking and to improve the quality of care and develop better treatments for the patients in the future.

Beyond the InfCare HIV system, the presentation will cover some examples of these treatment monitoring systems for other clinical specialties.

BMJ Open recently published an article describing the first 20 years with the Swedish InfCare HIV cohort and the main writer in the article, Christina Carlander (MD, PhD. Senior Consultant of Infectious Diseases at Karolinska University Hospital), is the main lecturer in this presentation.
