Theme: Supporting people with Long Term Conditions (LTCs) not only to manage their health, but to live their best lives is a huge global health challenge. Context: Personalised Care and Support Planning (PCSP) is the principle clinical method for delivering person-centred care for LTCs the UK. It is part of the Realistic Medicine programme in Scotland (2016)1 and Personalised Care in England (2019)2. Year of Care Partnerships (YoCP) have developed and delivered PCSP training and implementation in health communities in England and Scotland since 20113. Around 1:6 general practices in Scotland use this approach for people living with single and multiple LTCs. The National University Health Systems Cluster in Singapore have adapted and adopted PCSP for specialist clinics (2018) and polyclinics (2019) for people with diabetes. Impact: The YoCP approach to PCSP is robust and effective in diverse cultures within and across all three nations. It delivers person-centred care, promotes system changes that enable more equal partnerships between people with LTCs and healthcare professionals, provides greater enablement and self-efficacy, and improves health outcomes. Workshop: Participants will: Explore the values-base, clinical, economic and whole system benefits of PCSP, and case studies of impact. Engage in exercises derived from personal experience to understand the rationale for PCSP, explore the underpinning philosophical approaches and be able to relate these to clinical practice. Be introduced to the YoCP consultation model for PCSP. Reflect upon the core system components and skills of successful PCSP. Have the opportunity to test how these approaches can be applied in their own health setting.

Lindsay Oliver qualified in 1989 with a degree in Nutrition and Dietetics. In 2003, she became the first Consultant Dietitian in the UK and used the opportunity to become more involved in the development of self-care programmes both at a local and national level, including the development, research and roll out of both the DAFNE (Type 1) and DESMOND (Type 2) diabetes programmes .
In addition she has a major interest in communications skills and patient centred care, including the development of innovative approaches to patient care. in 2009 she was incited to head up the national training team for the Year of Care Programme, which aims to help people with long term conditions have more meaningful and useful conversations with health and care professional .
Nick Lewis-Barned Workshop facilitator

Nick has worked in the UK and New Zealand as a Diabetes and Endocrinology Specialist and Acute Physician since 1990, most recently in Northumbria Healthcare NHS Foundation Trust. He has been a researcher and teacher, clinician and Head of Service, and Royal College of Physicians Clinical Fellow for Shared Decision Making.
He has longstanding interests in service delivery, translation of research into practice and person-centred care. He is part of the team that piloted and now supports health communities to implement Personalised Care and Support planning in the UK. He has written articles about care planning, support for self-management and commissioning to support this
Tong Wei Yew Workshop facilitator

Dr Tong Wei YEW is a Senior Consultant at the Division of Endocrinology, National University Hospital (NUH), and Assistant Professor at Yong Loo Lin School of Medicine, National University of Singapore.
Recognising the challenges and complexity faced by individuals living with diabetes, Dr Yew champions the delivery of person-centred, integrated and individualised diabetes care. As Deputy Director for the Centre for Chronic Disease Prevention and Management, National University Health System (NUHS), he leads the Personalised Care and Support Planning program which spans across NUH, National University Polyclinics and Regional Health System, transforming traditional consultations into meaningful conversations, to promote shared decision making and better support for self-management. His work in this area was recognised with the NUHS-Mochtar Riady Pinnacle Team Award in 2022. He chairs the NUHS Patient and Family Advocacy Council, and is a WE CARE Arbinger Outward Mindset In-house Facilitator and People Leader for NUHS.
He also has keen interests in the area of gestational diabetes, diabetes in pregnancy and thyroid disorders in pregnancy. He was lead investigator for the SMART-GDM study, the largest randomised control trial using a smartphone coaching app among women with gestational diabetes, which showed better glycaemic control and neonatal outcomes. He was awarded the National Medical Research Council Individual Research Grant - New Investigator Grant in 2016, and the NUHS Clinician Scientist Programme Award in 2017.
On the education front, he is Program Director for the NUHS Endocrinology Senior Residency Program and previously core faculty member of the NUHS Internal Medicine Residency Program. He is a passionate teacher and has won numerous teaching awards.
He is the past treasurer and executive committee member of the Endocrine and Metabolic Society of Singapore. In his free time, he enjoys doing Chinese calligraphy, painting and seal carvings, and his artwork has been exhibited internationally. He also contributes regularly to columns in print media where he writes about heart-warming stories he has encountered in the course of his work.
Victor Loh Workshop facilitator
Family Physician, Medical Educator
National University of Singapore
Victor is Education Director of Family Medicine at NUS Medicine, National University of Singapore, and Senior Consultant Family Physician at National University Polyclinics.
He anchors the Motivational Interviewing and Communication with Person with Disabilities (PWD) curriculum at medical school. With the NUHS Personalised Care Support Planning (PCSP) team, he has been involved in training health and community partners in PCSP methodology, and in the implementation of PCSP as a platform for Shared Decision Making and Person Centred Care across Singapore.
His other academic interests include Humanistic Medical Professional Identity Formation, and Primary Care Antimicrobial Stewardship (PC-AMS).
