How can person-centred care become a key strategy to strengthen healthcare system resilience in crises, disasters, and within the context of total defence? Drawing on lessons from the pandemic, current research, and experience in preparedness planning, we show how person-centred approaches can help safeguard continuity of care and patient participation even under extreme conditions. By integrating person-centred principles into preparedness plans, more robust systems can be built to withstand pressure. The lecture offers practical insights and tools to apply in your own organisation.

In 2025, Primary Health Care Centre No. 4 (CCN4) in Kryvyi Rih, in partnership with ImagineCare, implemented a proof-of-concept (POC) Remote Patient Monitoring (RPM) project targeting patients with heart failure and hypertension—two of the most prevalent and high-risk cardiovascular conditions among its population. Using a digital health platform for real-time data collection, the initiative enabled continuous remote monitoring, early identification of health deterioration, and timely medical intervention. The project was designed to improve clinical outcomes, reduce emergency hospitalizations, and optimize healthcare delivery for patients with chronic conditions.A case presentation followed by a short panel on the learning opportunities through collaboration between Sweden and Ukraine
