Person-Centred Care places individuals and families at the heart of health and social care, ensuring services are kind, personalised, coordinated, and enabling. Supported by the NHS Charitable Foundation, NHS Tayside have implemented the Person-Centred Care Charter, a framework connecting these principles across all levels of service delivery. Underpinned by clinical governance, it drives organisational improvement and change.  The healthcare ecosystem comprises patients, staff, and the organisation. To strengthen this system, NHS Tayside combined concepts of Realistic Medicine and stakeholder consultation to design a unique self-assessment tool, that evaluates care and support improvement through four themes:  Right Person, Right Place, Right Time – Care is delivered with dignity, respect, and privacy. People co-design services, communication is tailored, and environments are welcoming and accessible.  Individual Experiences – Shared decision-making and personalisation are prioritised. Coordinated care and advocacy arrangements support individual needs, while education and resources empower self-management.  Teams with Skills and Capacity – Staff benefit from compassionate leadership, appropriate staffing, and equitable access to learning. Personal development enhances person-centred practice.  A System that Listens, Learns & Changes – Feedback, data-driven improvement, and a compassionate culture underpin continuous innovation. Teams share learning through networks to drive local developments.  Improvement areas identified through self-assessment are addressed over nine months, while elevated practices are evidenced and reviewed through clinical governance. This process recognises achievements and supports ongoing development.  The Charter aligns with national and international standards, reinforcing NHS Tayside’s commitment to sustainable, person-centred care. Acting as a bridge between practice, organisation, and governance, it ensures care is tailored to individual needs, delivered by a skilled workforce within a responsive system, and continually reviewed. This integrated approach fosters compassion, learning, and continuous improvement, delivering better outcomes for people across Tayside.
