
Smart Stroke Systems: Synchronizing Pre-Hospital and Intra-Hospital Workflows for Thrombectomy via Mobile Apps Bridging the Communication Gap Between EMS and the Angio Suite

Background: "Time is Brain" in acute ischemic stroke. Maximizing the number of patients who can be saved requires more than medical expertise; it demands optimized logistics. In 2021, Örebro University Hospital (USÖ) launched its thrombectomy service with a mission to revolutionize the standard of care through digital transformation.
Method & Implementation: To overcome traditional bottlenecks, USÖ implemented a "Smart Stroke System." We integrated AI software for automated interpretation of CT/CTA/CTP imaging, linked directly to a secure mobile application. This system sends real-time alerts and high-quality images to the on-call intervention team, enabling treatment decisions to be made within minutes of scanning, regardless of the physician's physical location.
Results: The integration of AI and mobile decision support yielded rapid results. In 2022, just one year after inception, USÖ treated 21% of all incoming ischemic stroke patients with mechanical thrombectomy—statistically the highest rate in Sweden. Despite the significant increase in volume and broader inclusion criteria, there were no signs of increased morbidity or mortality compared to national averages.
Conclusion: The experience at USÖ proves that integrating AI and mobile workflows is not merely a technological upgrade, but a clinical necessity for identifying eligible patients who might otherwise be missed. In this session, we will share our workflow strategies, the role of AI in "democratizing" stroke expertise, and how digital health tools directly translate into saved brain tissue.
