Hur går man från en lokal idé till ett datadrivet verktyg som används i hela regionen – och dessutom sprids vidare till andra? Region Västmanlands vårdtyngdsverktyg har på bara några år utvecklats från prototyp till ett verksamhetskritiskt beslutsstöd i den dagliga vården.
Arbetet och verktyget har uppmärksammats av både Socialstyrelsen och Socialdepartementet som delade ut Svea Priset till Region Västmanland 2025.
I denna föreläsning berättar vi öppet och konkret om hela resan: - hur behovet identifierades - hur lösningen utvecklades tillsammans med verksamheten - hur införandet genomfördes på samtliga vårdavdelningar - och hur vi nu sprider verktyget vidare genom avtal och samarbete med Region Östergötland.
