Multimorbidity, the coexistence of two or more chronic conditions, represents a pressing challenge for healthcare systems in Europe as populations age and the prevalence of complex health conditions rises. Current healthcare models, which are often siloed and reactive, fail to meet the interdependent needs of multimorbid persons (PMM). The CareMatrix Challenge under the EU Horizon 2020 framework seeks to address these gaps by fostering the development of scalable, patient-centered, and interoperable solutions.The INCA (Integrated Care) solution, developed by a consortium of clinical, academic, and technological partners, offers a comprehensive response to this challenge. Built on a telemedicine platform provided by Doole Health, INCA combines clinical expertise from Fight Infections Foundation and Germans Trias i Pujol Research Institute with academic contributions from the Open University of Catalonia and the Norwegian Centre for E-Health Research. Together, the consortium has created a solution that emphasizes interdisciplinary collaboration and patient empowerment.The INCA platform consists of a mobile app for patients and caregivers and a web-based interface for healthcare professionals. Features include tailored questionnaires for early identification of PMM, an electronic health and social record, shared care plans, a messaging system, and a clinical decision-support system. The platform’s gamified tools promote adherence to treatment and healthy habits, while its multichannel communication framework ensures seamless coordination across care levels. A standout capability of INCA is its ability to integrate diverse data sources, including social and behavioral information, enabling a holistic overview of patient health.Compliant with the principles of the European Health Data Space, INCA ensures secure, high-speed data exchange while leveraging an open-source infrastructure for interoperability. The platform’s flexibility allows for customization to meet the unique requirements of various healthcare systems, legal contexts, and care pathways, facilitating deployment across diverse settings.To validate its usability, functionality, and impact, the INCA solution will undergo clinical testing at three European sites: Region Skåne (Sweden), Vestre Viken (Norway), and Osakidetza (Spain). These evaluations will assess user experiences from patients, caregivers, and healthcare professionals, providing essential feedback to refine the solution and demonstrate its potential to transform care for multimorbid populations. Through its holistic approach and innovative design, INCA is well positioned to address the systemic challenges of multimorbidity and contribute to sustainable, integrated care.

Title: Integrating standardized nutrition terminology into electronic health records.Over the last two decades of inception, Nutrition Care Process and Terminology (NCPT) has evolved into a globally adopted standardized language model for providing critical thinking and decision-making framework that facilitates good nutritional care practices and supports impact assessment. The growing need for medical data entry in structured or pre-defined formats in Electronic Health Record (EHR) System has fostered the development of NCPT reference sets in SNOMED CT. This lecture will share the outcome of the INIS (International Nutrition Care Process and Implementation Survey) study in Scandinavian countries which examined how standardized nutrition terminologies are integrated into the EHR systems.

The presentation describes the potential of remote monitoring of vital physiological signs as part of supporting the independent living of older adults. A study was conducted in which 10 seniors measured their vital physiological signs over a 2-month period using a mobile application. Participants found the measurement easy and useful, but challenges arose in interpreting the results. Learn more, what is important when developing remote technologies for seniors.

The ASAP Stroke project aims to improve prehospital stroke care by leveraging artificial intelligence (AI) for early assessment and triage. The project focuses on integrating diverse data sources, such as registry data, videos, and vital signs, to create predictive models that identify stroke, distinguish large vessel occlusions and guide optimal care pathways. The project also explores synthetic data generation to address privacy concerns. By improving early stroke assessment and patient triage, ASAP Stroke aims to bridge the gap between prehospital and in-hospital care, ultimately improving treatment outcomes and reducing time-to-treatment.

The rise of closed-loop systems in healthcare introduces significant cybersecurity challenges, as these devices autonomously measure, analyze, and act on physiological data. Key risks include vulnerabilities in sensors, actuators, and control algorithms. This presentation explores the regulatory frameworks of the EU Medical Device Regulation (MDR) and U.S. FDA, with a focus on FDA guidelines for physiological closed-loop controlled devices. A real-world case study highlights the challenges in securing these systems. Practical recommendations are provided to help manufacturers address evolving threats and ensure safety and reliability throughout the device lifecycle.

Asthma is a devastating disease affecting more than 334M people in the world. 40-60% of them suffer from moderate to severe asthma of which 30% would benefit from additional remote monitoring and/or digital health tools. It has been shown in literature that easy algorithms can improve asthma control by 30% by increasing therapy adherence. Artificial intelligence opens up the door towards risk prediction and self-learning medication and therapy adaptations. This session will specifically zoom in on the potential of AI in predicting risks for exacerbations and loss of asthma control. Moreover we will shed lights on the potential of novel (digital) biomarkers to drive AI-based risk prediction. Lastly, the efficacy of all digital tools is limited to their reach into patient engagement. How to keep the patients and clinical team onboarded? Can we calculate the impact from the tools to what it means in terms of savings for healthcare systems? And how this is affected by patient engagement? Can we formulate some guidelines towards future developers and policy? Treatment is good but prevention is better, and for that we need the tools, the engaged patients and a favorable environment. The session will share insights of recently published data, next to the announcement of a promising clinical trial that will start in Belgium and the Netherlands in Q2 2025.
