We present a system MyVersion where the patients reports PREM- Patient Related Experience Measurements both via the medical history and validated questionnaires. It will also manage PROM- Patient Related Outcome Measurements. It is built with international standards regarding SQL database, FHIR and API as well as standards for identification and security. The technical solution will be presented, and we will elucidate the system in four perspectives: Patient: I can present my symptoms in a structured way, and be sure the caregiver notice them and I can control that they are admitted correctly to my medical report. I am guided through the process and see the results. Caregiver: I do not need to shift system, but can see all relevant data and results from questionnaires and auto-anamnesis in my ordinary medical record system, complete and correct, sign and give my time to the patient, not the computer. Researcher: I do have access to validated data in a local or national database, structured, safe and anonymized Public: We can follow and evaluate clinics, patient cohorts and processes in a way that do not reveal sensitive or personal data. Some technical language will be used, but the presentation should be easy to follow for anyone interested.
