Policy-practice gap in person-centered care for maternal mental health in India [PCC065]

Background: Person-centered care (PCC) is a keystone of quality maternity services, but its role in maternal mental health (MMH) in low- and middle-income settings is under-explored. India’s policies advocate respectful maternity care, yet implementation gaps limit integration of MMH. Objective: To identify policy presence of PCC elements related to MMH and correlate it with implementation realities and suggest India-specific priority actions to address existing gaps. Methods: We followed JBI scoping review methods, searching seven databases and grey literature for 2015–2025. Data from 25 records (empirical studies, policies, guidelines) were charted and thematically analysed; global benchmarking and contextual gap assessment informed framework development. Results: While Indian policy documents support multiple several PCC domains like dignity, respect, privacy, equity, and birth companions as evident in policy guidelines like SUMAN, LaQshya; its implementation is inconsistent. Major gaps include poor empathetic communication and lack of MMH training, weak shared decision-making, absence of formal emotional/psychological support in ante-natal care (ANC) & post-natal care (PNC), fragmented continuity of mental-health follow-up, inadequate confidentiality for mental health concerns, and limited culturally adapted interventions. Discussion: The review reveals a clear policy–practice gap: structural commitments exist but MMH is not operationalised within routine PCC. Priority actions for an India-specific adaptation framework include integrating MMH screening and psychosocial support into ANC/PNC, training staff in empathetic communication and shared decision-making, strengthening confidentiality, monitoring and accountability mechanisms, and co-developing culturally and equity-sensitive interventions. Focusing on these enablers can translate policy into measurable improvements in maternal mental health and person-centred maternity care.
